Doctor-led approach

Clinical overview

CoolSculpting uses controlled cooling to target and reduce fat cells in specific areas of the body. The technology, known as cryolipolysis, works by cooling fat cells to a temperature at which they undergo apoptosis (programmed cell death), while the overlying skin and surrounding tissue remain unaffected. The body then gradually clears the treated fat cells through its natural metabolic processes over the following weeks and months. CoolSculpting is designed for localised fat reduction in patients who are close to their target weight but have stubborn fat deposits that have not responded to diet and exercise. It is not a weight-loss treatment and is not appropriate for patients with obesity. Common treatment areas include the abdomen, flanks, upper arms, back, inner and outer thighs, submental area (chin), male chest, and bra fat. Most patients require one session per treatment area. Results build progressively over 2–3 months as treated fat cells are cleared. A known but rare risk is paradoxical adipose hyperplasia (PAH), where the treated area increases in size rather than reducing. This is discussed during consultation. Dr Langdon is a member of the CoolSculpting Global Advisory Board and Allergan CoolSculpting Faculty, and serves as a UK complications expert for the technology.

When we don't recommend it

  • If the concern is overall weight rather than localised fat deposits — CoolSculpting is not a weight-loss treatment and will not address generalised excess weight.
  • If you have a cold-sensitivity condition (cryoglobulinemia, cold agglutinin disease, paroxysmal cold haemoglobinuria) — cryolipolysis is contraindicated.
  • If the treatment area has active skin conditions, hernias or recent surgical wounds — treatment should be delayed until the area is safe.
  • If expectations are for immediate visible results — fat clearance is a gradual process over 2–3 months and patience is required.
  • If a thorough assessment suggests that the fat deposit would respond better to a different approach — we will discuss alternatives honestly.

What Happens During Treatment

Risks & Side Effects

Common, Usually Temporary

  • Temporary redness, swelling or bruising in the treated area
  • Numbness or tingling (usually resolves within hours to days)
  • Temporary firmness, cramping or aching in the treated area
  • Skin sensitivity at the treatment site

Rare but Serious

  • Paradoxical adipose hyperplasia (PAH) — a rare condition where the treated area enlarges rather than reduces; this is discussed during consultation
  • Prolonged numbness or nerve-related discomfort (usually self-limiting)
  • Late-onset pain in the treated area (rare, typically resolves)
  • Skin changes such as frost-bite-like appearance (very rare with correct applicator use)

When to Contact Us Urgently

  • The treated area becomes firmer, larger or more prominent rather than reducing over time
  • Severe or prolonged pain that does not settle with simple analgesia
  • Significant skin changes (blanching, blistering or discolouration) in the treated area
  • Any symptom that concerns you or worsens rather than improves
