What is PAH after CoolSculpting?
Paradoxical adipose hyperplasia, or PAH, is a rare complication of cryolipolysis where the treated tissue gradually enlarges rather than reduces, typically becoming noticeable between two and nine months after treatment. It is not weight gain and does not resolve spontaneously. Management is doctor-led and may include monitoring, structured assessment and onward referral for liposuction correction where appropriate.
Book a consultationWhen this may help
- You have noticed an enlarging, well-defined area in a previously treated cryolipolysis zone.
- You want assessment from a clinician with specific CoolSculpting complications experience.
- You want to understand realistic management options before deciding next steps.
When this may not be suitable
- Your concern is generalised weight gain rather than a discrete enlarging area.
- You are looking for a guaranteed same-day correction without assessment.
- You need urgent care for an unrelated acute medical problem.
Risks and limitations
- PAH does not resolve on its own and may need surgical correction such as liposuction.
- Delaying assessment can make the area harder to treat aesthetically.
- Surgical correction carries its own anaesthetic, bruising, swelling and scarring risks.
- Outcomes after correction vary by individual.
What we assess in consultation
- History of original cryolipolysis treatment and timeline of change
- Examination of the affected area and comparison with surrounding tissue
- Discussion of monitoring, correction options and onward referral
- Honest conversation about expectations and what correction can achieve
