Berkshire Aesthetics is a doctor-led, CQC-registered aesthetic clinic in Maidenhead. All treatments begin with a consultation to assess suitability, medical history, risks and realistic outcomes.

What causes hollow under-eyes?

Hollow under-eyes are usually caused by a combination of factors: descent or prominence of orbital fat pads creating a tear-trough depression, mid-face volume loss, thinning of the periorbital skin with ageing, and genetic anatomy. Sleep deprivation, dehydration and pigmentation can make hollows look worse. Treatment depends on the dominant cause and is decided in a doctor-led consultation.

When this may help

  • You want to understand whether your hollows are anatomical, age-related or lifestyle-driven.
  • You are unsure whether tear-trough filler, skin boosters or no injection is most appropriate.
  • You want a careful assessment of a delicate, higher-risk area.

When this may not be suitable

  • You expect a guaranteed result from a single treatment in a complex area.
  • You have active infection, significant under-eye bags from herniated fat needing surgical opinion, or relevant contraindications.
  • You are pregnant or breastfeeding.

Risks and limitations

  • The under-eye area carries specific vascular and swelling risks with injectables.
  • Tyndall effect, prolonged swelling, lumps or migration can occur with filler if poorly placed.
  • Surgical referral may be more appropriate than injectable treatment for some patients.

What we assess in consultation

  • Whether hollowing is from fat descent, volume loss, skin thinning or pigmentation
  • Whether tear-trough filler, polynucleotides, skin boosters or no treatment is safest
  • Medical history, allergies and previous injectable reactions
  • When surgical referral is the more honest option
Unsure whether this is right for you? A consultation is the safest next step.

Related questions

Are dark circles the same as hollows?

No. Hollows are a contour issue caused by anatomy and volume loss, while dark circles can also be caused by pigmentation or visible vasculature. The two often co-exist.

Will sleep and hydration fix hollow under-eyes?

They can help reduce how tired the area looks but rarely change underlying anatomy or volume loss.

Is filler always the answer?

No. The under-eye is a higher-risk area and not every patient is suitable. Polynucleotides, skin boosters or surgical opinion may be more appropriate.
